Liability Waiver & Photo Consent

I understand that participation in Yarns & Tiles Summer Camp involves hands-on creative activities including, but not limited to, mosaic art (tile cutting, grout, adhesives), rug tufting (tufting gun, yarn), weaving, terrarium building (soil, small tools), and pressed-flower framing (scissors, heat tools).

I acknowledge that these activities carry certain inherent risks including minor cuts, abrasions, skin or eye irritation from materials, and other injuries that may arise despite reasonable precautions. I voluntarily accept these risks on behalf of my child.

To the fullest extent permitted by California law, I hereby release, discharge, and hold harmless Yarns & Tiles, its instructors, staff, volunteers, and agents from any and all claims, demands, losses, or damages arising from my child’s participation in camp activities, except where caused by gross negligence or willful misconduct on the part of Yarns & Tiles.

I confirm that the health and allergy information provided during registration is accurate and complete. I authorize Yarns & Tiles staff to seek emergency medical treatment for my child if I cannot be reached and a medical professional determines it is immediately necessary. I agree to bear all associated medical costs.

I understand that Yarns & Tiles staff will supervise all camp activities; however, I acknowledge that constant one-on-one supervision is not possible in a group setting. I agree to ensure my child is picked up promptly at the stated end time each day. Children may not be left unsupervised before or after program hours.

I understand that Yarns & Tiles reserves the right to dismiss a child from the program for behavioral or safety reasons, without refund.

I grant Yarns & Tiles permission to photograph and/or video record my child during camp activities for use in promotional materials, website content, and social media — at no cost to Yarns & Tiles. Images will not be sold to third parties.

If you do notwish your child to appear in photos or videos, please indicate this in the “Anything else we should know” field during registration. We will ensure your child is excluded from all media.

By checking the waiver acknowledgement box on the registration form, I confirm that I am the parent or legal guardian of the child(ren) being registered, I am at least 18 years of age, and I have read, understood, and agree to all terms above. This digital acknowledgement carries the same legal weight as a handwritten signature under California’s Uniform Electronic Transactions Act (UETA).
